The incident report [assault with intent to commit murder] indicates of shots fired at police, victim shot, chasing criminal and shooting him. There are three other officers named [Johnstone, Lasher and Kirby] in the report about this incident. The incident took place at the Northern Station District Compl E-56. The victim was found near Haight and Fillmore Streets. [Suspect appears to have escaped while shot and arrested later]. The victim and shooter is named in this report. Official, Signed Certificate is 10" x 12-3/4". The pink copy of the official report [unsigned] is 11" x 8-1/4". Both items remain in very good condition.
